You can participate in-store or online:

Option 1: In-Store Entry

  • Complete the Made for You Giveaway Entry Form at the register.

  • Spend $15 or more in the shop (food, drinks, or products) and receive 2 entries automatically submitted for you.

Option 2: Online Entry

  • Fill out the Giveaway Entry Form below.

  • Limit 1 online entry per person.

🌈 Who Can Enter

This giveaway is designed for individuals ready to experience Food as Medicine through a guided detox and nourishment reset.

✅ Must have a diagnosed chronic condition (see list on entry form: diabetes, hypertension, obesity, GI disorders, autoimmune, etc.).
✅ Must be insured and give permission for Farm 2 Farmacy to submit for reimbursement through a superbill or have an active FSA/HSA card.
✅ Must be willing to share your journey publicly on social media and/or provide testimonials to help inspire others in our community.

A 1-Week “Made for You: Flex ROOT Plan” — a medically tailored Food-as-Medicine experience designed by our dietitian and culinary wellness team.

Includes:
🥗 3 Personalized Meals (per week for 1 week)
🥤 1-Day Whole-Food Juice Cleanse — 6 × 18 oz juices
🌿 Detox Protocol Kit, including:

  • Herbal Detox Cleansing Tea (to start before your cleanse)

  2. Completion of your superbill or FSA/HSA authorization

  3. Photo or short video consent (before/during/after experience)

  4. Check-in + follow-up support with the Farm 2 Farmacy team
